2nd ESO - School vaccination. Information, authorization and calendar

Dear 2nd grade families from ESO,

We are sending you a letter (Vaccination information 2022-23) from the Public Health Agency of the Generalitat de Catalunya addressed to families with daughters in your course.

In Les Alzines, these vaccines will be administered on Tuesday 28 February from 9am, by the CAP Montilivi de Girona.

In order to prepare the documentation that we must deliver to the CAP, we ask that, before February 14th, you send your daughter's guardian a photocopy of the page of the vaccines she has received so far (if not contains your daughter's name, put it by hand, please), along with the authorization sheet (Vaccination Authorization) for the vaccines you want to be administered at school, which has to be signed.

The vaccination book must be delivered to the tutor on February 27, so that the nurse in charge completes the corresponding section. This book will remain in the possession of the school until the moment of vaccination, and we will return it to you once the students have received the vaccine. Only those who have the paternal/maternal authorization together with the vaccination book will be vaccinated.
