2º ESO - School vaccination

Dear families:

  We send you a circular letter from the Public Health Agency of the Generalitat de Catalunya aimed at families with sons and daughters that are in 2nd ESO. 

  This vaccine will be administered in Les Alzines in April, in charge of CAP Montilivi of Girona. Afterward we will confirm the exact date. 

  To prepare the documentation that we have to deliver to the CAP, we ask you that, before Monday, February 10th, you give to your daughter's tutor a photocopy of the vaccines page that she has received so far - if your daughter's name is not recorded write it by hand - along with the sheet with the authorization of the vaccines that you want to be administered at school. 

  The vaccination book must be delivered to the tutor of your daughter before the day of vaccination, so the nurse in charge will complement the corresponding section. This book will remain in possession of the school until the moment of vaccination and we will return it once the students have received the vaccine. 

 Only students who have parental authorization along with the vaccination book will be vaccinated. 

  For any information about these vaccines, you can attend to your daughter’s pediatrician.
