2nd of ESO - School vaccination. Information, authorization and schedule

Dear 2nd year ESO families, 

Here you have the circular from the "Public Health Agency of the Generalitat de Catalunya" (click here) addressed to families with children in the 2nd year of ESO. In Les Alzines this vaccine will be administered in January 2021, by the CAP Montilivi of Girona. We will confirm the exact date later.

To prepare the documentation that we have to deliver to the CAP, before November the 30th, you should provide your daughter's tutor with a photocopy of the vaccination page (if your daughter's name does not appear, write it down by hand ) that you have received recently, along with the authorization sheet for the vaccines you want to be administered at school, which must be signed.

The vaccination book will be delivered to the tutor before the vaccination day, so that the nurse in charge can complete the corresponding section. This book will remain in the school until the moment of the vaccination, and we will return it once the students have received the vaccine.

Only female students who carry parental authorization (click here) along with the vaccination book will be vaccinated.
